Job Description

Title: Aftercare Substance Abuse Counselor (evening)

Department: Ambulatory Services

Reports to: Director of Ambulatory Services

Requirements: Masters degree in social work or related field with one year experience or Bachelors degree in human services or related field with three years substance abuse case management and/or counseling experience or five years case management /counseling experience. Bilingual (English and Spanish) preferred. CASAC or CASAC-Trainee required.

Duties*:

  • Provide individual and group counseling services to clients who are working, attending school and/or vocational training.

  • Maintain a caseload of 15-25 aftercare/out patient clients.

  • Conduct intake and psychosocial assessment of clients.

  • Maintain documentation of client case record according Greenhope, OASAS and all other contracts and requirements.

  • Participate in case reviews and case conferences.

  • Conduct monthly home visits with portion of caseload.

  • Coordinate and provide outreach and follow-up services (via telephone, letters, etc.) to clients as necessary.

  • Facilitate relapse prevention and other life skills groups.

  • Work with the interdisciplinary team members in addressing issues of dysfunction as a result of drug/alcohol dependence/abuse, coordinate services with other departments, formulate treatment plans and communicate to team about client treatment progress.

  • Maintain communication and coordinate services with housing, medical and other social service providers as necessary.

  • Provide literature, referrals and follow-up services to clients.

  • Assist with data collection to generate Aftercare reports on a monthly basis.

  • Keep abreast of developments in the field of chemical dependence/abuse, HIV/AIDS, criminal justice reentry, welfare reform laws, women issues, and family development via, meetings, conferences, training and literature.

  • Other duties as assigned by supervisor.
